Our 18-month-old Cocker Spaniel, Butterscotch, becomes completely unhinged at the vet. Trembling, panting, won't let the vet examine him, bit the vet assistant once (didn't break skin but still). He's perfectly calm everywhere else — it seems entirely vet-specific.
We go to a good clinic in Bengaluru that uses positive reinforcement approaches, but Butterscotch is in such a high-anxiety state by the time we arrive that no amount of treats helps — he won't even take his favourite food at the vet.
The vet has suggested "Happy Visits" — going to the clinic just for a treat and to hang out — which we haven't started yet. Is this actually effective? And are there specific things to train at home that mimic vet examination handling?
Happy Visits are genuinely one of the most effective things you can do for vet anxiety and I can't recommend them enough. The principle is counter-conditioning: you're building a new emotional association with "vet clinic = good things happen." Over 8-12 Happy Visits (2 per week, 10 minutes each — go in, treats, pets from staff, leave), many dogs show dramatic reduction in anxiety.
But the key is his arousal threshold at arrival. He needs to arrive calm enough to take treats. Start by just sitting in the car park of the clinic, give treats, drive away. Next week, walk to the door, treats, leave. Build up very gradually.
We did Happy Visits for 8 weeks before Butterscotch's next actual appointment. The difference was remarkable — he still wasn't happy to be there but he took treats from the vet assistant for the first time ever. That was a massive win.
The "sitting in the car park for treats" starting point sounds too small to matter but it genuinely worked as a first step for our anxious rescue. Start smaller than you think you need to.
Home handling exercises that replicate vet examination: daily "vet prep" sessions where you gently handle his ears, open his mouth, look at his teeth, touch his paws and toenails, press on his abdomen lightly, and run hands along his spine — all while giving treats. Call it anything except "vet exam" in your head so your own anxiety doesn't transmit to him.
Do this every single day for 10 minutes. After 4-6 weeks, the handling itself becomes a neutral experience and the vet doing it is less alarming.
The daily handling routine became a calming ritual for our Cocker — he now actually brings his paw to us for inspection. Started as vet prep and turned into a bonding activity. Unexpected bonus.
For the high-anxiety threshold on arrival — talk to your vet about a pre-visit sedative (gabapentin or trazodone are commonly used). Given 2 hours before the appointment, they reduce anxiety enough for the dog to accept treats and be examined without trauma. Not for every visit but for any invasive procedure or when anxiety is currently this severe.
Also consider the travel to the vet as part of the anxiety chain. If the car ride is associated with only vet trips, the anxiety builds from the moment he gets in the car. Take him on fun car trips to the park, for drives with no destination, at least once for every vet visit. Break the "car = vet = terrifying" chain.
Butterscotch not taking treats at the vet is a common sign that he's over threshold — when cortisol and adrenaline are high enough, dogs stop being food-motivated. You're trying to counter-condition in a state where it can't work. The Happy Visits + gradual desensitisation approach is about bringing that threshold down first.
Two high-value treats that work even over threshold in many dogs: real cooked chicken (not kibble, not commercial treats) and real cheese. The smell cuts through the anxiety in a way that processed treats don't. Bring Butterscotch's absolute favourite food, something he only ever gets for this specific purpose.
